How is Anchored Pathways different from hospice?

Anchored Pathways does not replace hospice care. Instead, it provides an additional layer of support and education for families. Drawing on years of hospice nursing experience, I help families understand the process, prepare for what lies ahead, and feel supported every step of the way.

What is hospice care?

Hospice care is specialized care focused on comfort, quality of life, and symptom management for individuals facing
a life-limiting illness. The goal is to provide physical, emotional, and spiritual support to both patients and their families.

When is it time to consider hospice?

Hospice may be appropriate when a person's illness is no longer responding to curative treatment, or when the burdens of treatment outweigh the benefits. Signs may include frequent hospitalizations, increasing weakness, weight loss, declining function, or a desire to focus on comfort and quality of life.

Does choosing hospice mean giving up?

No. Hospice is not about giving up. Hospice focuses on comfort, dignity, and quality of life while supporting both the patient and family through the journey.

What services does hospice provide?

Hospice typically provides nursing care, physician oversight, medications related to the terminal diagnosis, medical equipment, social work services, spiritual support, bereavement services, and trained volunteers.

Where is hospice care provided?

Hospice care can be provided in a patient's home, assisted living facility, nursing home, hospice residence, or inpatient hospice unit, depending on the individual's needs.

Who pays for hospice?

Most hospice services are covered by Medicare, Medicaid, and many private insurance plans. Coverage often includes medications, equipment, and supplies related to the hospice diagnosis.

A Hospice Navigation Session provides personalized guidance for families who are considering hospice or currently receiving hospice care. Together we discuss what to expect, available options, important decisions, communication with the hospice team, and ways to navigate the journey with greater confidence and clarity.

Can you help if my loved one is not on hospice yet?

Absolutely. Many families reach out while exploring whether hospice may be appropriate. I can help you understand hospice eligibility, discuss concerns, and prepare for conversations with healthcare providers.

Can family members participate in consultations?

Yes. Family members are encouraged to participate. Hospice decisions often affect the entire family, and having everyone involved can help improve understanding and communication.

Are consultations available virtually?

Yes. Consultations are available in person, by phone, or virtually, allowing families to receive support in the way that is most convenient and comfortable for them.

What if my family disagrees about hospice?

What happens during the final stages of life?
