For patients going into hospice or who are already on hospice. This package offers ongoing support with unlimited communication via text, email, phone calls, and/or video calls. To help with continued support in communicating with your hospice team, understanding updates, medications, changes in condition, comfort support and preparing for the final stages. All of this in everyday language.
Once again, I am the founder of Anchored Pathways, and you'll be speaking directly with me every time. No hand-offs, no guessing, no rotating staff. Just consistent, one-on-one support from me, who will already know your story.
